Impact of socio-economical inequalities in the progression of HIV infection at individual and contextual level in Europe

Description
'Background. The HIV epidemic is a major public health issue in Europe. Over the last decade significant changes in the epidemic have taken place with a increasing proportion of HIV infections being in migrants and women. In turn, these groups might not fully benefit from life-saving antiretroviral treatments (ART), because of barriers to HIV testing and to ART. To date no study with sufficient sample size has explored the role of socio-economic determinants in heath of HIV infected patients in Europe. The role of contextual factors in the health of HIV patients, such as ethnic density, and lower area income level, have not been studied. Objective. We aim to study the socio-economic determinants of inequalities in HIV diagnosis, disease progression, and treatment initiation in Western Europe. Moreover, we will explore the extent to which the variability of HIV prognosis is attributable to either individual (gender, ethnicity, social class) or contextual factors (income area, ethnic density). We will use the routinely collected data of HIV infected patients that will be merged by the newly established EuroCoord collaboration. Methods. The routinely collected database from the newly established EuroCoord collaboration provides information on socio-economic determinants on 250,000 HIV infected individuals from various EU countries. HIV disease progression will be defined as rate of CD4 decline or time to AIDS or death. Marginal structural models to account for time varying confounding will be used to explore differences by socio-economic groups of the effect of ART on the risk of AIDS and mortality. Multilevel and latent variable models will be used to explore the role of contextual factors on HIV progression. Public health implications. This research will help EU health care planners to identify and target groups of HIV infected individuals likely to face barriers to HIV testing and ART and thus prevent HIV-related mortality in more vulnerable populations.'

Gesundheitsversorgung von Menschen ohne legalen Aufenthaltsstatus in Deutschland ist defizitär. Ärzte berichten, dass diese, wenn überhaupt, erst zu spät medizinische Hilfe in Anspruch nehmen. Oft wird die Chance einer frühzeitigen Diagnose und Behandlung vertan. Der Rechtsanspruch auf ärztliche Behandlung nach dem Asylbewerberleistungsgesetz wird von Menschen ohne Papiere aus Furcht vor Statusaufdeckung selten eingelöst. In Deutschland haben sich parallele nichtstaatliche Strukturen der medizinischen Hilfe herausgebildet, die als primärmedizinische Anlaufstellen und Vermittlungsstellen fungieren. Eine interdisziplinäre Arbeitsgruppe der Universität Magdeburg und der Charité Berlin soll mit Hilfe qualitativer und quantitativer empirischer Methoden einen systematischen Überblick über Angebote nichtstaatlicher Angebote medizinischer Hilfe für Menschen ohne Papiere erarbeiten. Dabei wird fokussiert auf: gesundheitliche, sozialmedizinische und rechtliche Probleme in der Primärversorgung; Schnittstellenprobleme zu unterschiedlichen Leistungsanbietern im Gesundheitswesen; Barrieren im Zugang zu notwendigen Gesundheitsleistungen.

Coordinating resources to assess and improve health status of migrants from Latin America

European health systems are committed to meeting the challenge of understanding the needs of migrant populations and adapting their services to meet these needs. The difficulties inextricably linked to this challenge are caused by the complexity of migration patterns and the differences between migrant population across EU countries. At present, the limited available data show that attempts to incorporate migrants’ health needs, in particular those of migrants from non-EU countries, into EU health systems have remained scattered and uncoordinated. COHEMI’s general objective is to coordinate referral centres dealing with specific Latin American (LA) diseases in order to provide a clear understanding of the full migration cycle in relation with the health systems in Europe and Latin America and to provide a in-depth insight into priority health-related aspects of LA migration in order to facilitate the development of and transfer of evidence and information relevant to migrant health policies.

Au cours des dernières années, la composition des flux migratoires depuis et vers le Sénégal a connu de profondes évolutions, notamment en raison de la présence croissante de migrants hautement qualifiés. La crise économique, sociale, politique, etc. et la faillite des services sociaux consécutive aux programmes d’ajustement structurel ont considérablement alimenté ce phénomène. L’émigration des Sénégalais hautement qualifiés s’explique en partie par la recherche de meilleures conditions de travail et de salaires plus élevés. Parallèlement, la profonde instabilité politique de la région a précipité l’arrivée massive au Sénégal de migrants hautement qualifiés originaires d’autres pays africains. Ainsi, au Sénégal comme dans les autres pays de départ, la migration hautement qualifiée s’est accentuée - en dépit des efforts mobilisés par les politiques - et concerne désormais tous les secteurs professionnels (santé, enseignement, etc.). In the last years, migratory flows to and from Senegal have taken on new forms . In particular, the migration of highly-skilled individuals has become significant, in the wake of profound political and economic crises and particularly following on from the collapse of the country’s social services provoked by extensive structural adjustment programs. The departure of highly-skilled Senegalese nationals is largely due to economic push and pull factors. But chronic political instability in the region has also brought into Senegal substantial numbers of highly-skilled workers from other African countries. Consequently, highly-skilled migration is on the rise in Senegal as well as in other sending countries and, despite various policy initiatives, it is to be found along the whole spectrum of professional activities (health, teaching, etc.).
