Native-Born Youth of African Immigrants and Cardiovascular Risk: A Mixed Methods Study

Description
Cardiovascular risk factors (CR) are the leading cause of morbidity and mortality in developed countries. By adolescence, CR of diet, exercise, blood pressure, body mass index, and smoking already are predictive of adulthood morbidity. Although genetics play a part in CR, health behaviors are the major contributors. Health status and service use differ between ethnic/racial groups (including immigrants) and the dominant population, but is this true of the influences on health behaviors? Europe and Israel are common destinations for international migrants from the least developed African nations. Capitalizing on a mixed-methods design and Community Based Participatory Research approach, this study will address ethnic/racial disparities in health care access by examining the influences of all four Bioecological Model's domains on cardiovascular risk factors in youth, comparing native-born youth of African immigrants (Ethiopians) to veteran Israelis. Qualitative interviews from 45 stakeholders and youth will be used to identify unique influences not found in existing literature. Informed by the interviews, quantitative surveys with new and established instruments will measure the impact of the four domain influences of the Model on CR. The surveys will be administered in three areas of Israel to a sample (n=600) comprised of two groups of 15-18 year old native-born youth, one born to African immigrants and the other to veteran Israeli parents. Using a variety of statistical techniques, the impact of each domain will be measured for all youth and each group. This study represents the first attempt to integrate the full range of domains on health behaviors affecting CR in youth, and disaggregate them for dominant society and second generation immigrant youth. Not only will this information inform the development of effective health care interventions for all youth, but it also will reduce ethnic/racial disparities for Europe's future generations of immigrants.

"The ExiTT project (in its preparatory phase from January 2018 to December 2019) prepares a large-scale research project that processually traces, documents and analyses processes of migration from origin countries, from the starting point, over the route, until integration in the destination countries and their societies from social, economic, political and cultural aspects. At the same time, it will analyse the political, economic, societal, discursive and legal transformation of the sending, transit and receiving/destination countries as well as political, economic, societal, discursive and legal repercussions by, for example, transnational relationships, re-migration, circular migration or post-migration mobility. To grasp such complex processes, it employs a research approach that is interdisciplinary, multi-local and multi-method. The multidisciplinary research approach is intended to apply research perspectives from sociology, political sciences, psychology, geography, history, cultural sciences and economics in order to design questionnaires, to conceive the regional case studies and to carry out the data analyses. The main task of the project will be the collection and analysis of new data in multiple locations such as origin, transit, and receiving countries that will provide a unique basis to deliver evidence for the above sketched topics. The basic idea and method will follow a combination of the ethnosurvey model (Massey & Zenteno 2000) and regional case studies. On the basis of multiple methodological approaches such as surveys, fieldwork, discourse, media and policy analyses in origin, transit and immigration countries, the ExiTT project is intended to answer questions about the causes and motives of migration decisions, about the negotiations and conditions for successful integration and participation in transit and immigration countries, and about various aspects of social, political, economic and cultural transformations in all countries involved. The results of the data generated though this mixed methods approach is intended to be collated into one comprehensive data set that – based on the model of the Mexican Migration Project – will continue to grow cumulatively over time. In addition to surveys and (qualitative) interviews with individuals (migrants as well as non-migrants) expert interviews with representatives from state and private organisations as well as observations in the field will be conducted and innovative methods like experiments will be deployed. From the data set, there will derive a potential for research into the causes, conditions and negotiations of migration and migration routes as well as the changes in the so-called transit or host/receiving countries and their societies. At the same time, an interdependent approach will be chosen that includes in the analysis the effects of migration in the exit or origin countries. Insights into migration and integration processes from various actors and in multiple sub-systems can be expected on the basis of this data set. Labour market-specific and education-related aspects, cultural and social practices of integration, and obstacles and negative effects of disintegration could be evaluated according to target groups and, e.g., analysed in families and house-holds from the gender perspective or with regard to youths. In parallel, transformations in political structures, cultures or societies can be documented and the analysis of changes empirically grounded. The ExiTT project is a cooperation project of the DeZIM research community."

"Ziel des Kooperationsprojektes „RADIKAL“ ist das Verständnis des Zusammenhangs von Migration, Religion und Radikalisierung. Zentral ist dabei die Analyse der Beziehung zwischen Migrationserfahrungen, Akkulturationsprozessen, Diskriminierungswahrnehmungen und -erfahrungen und der Bildung von politischen wie religiösen Überzeugungen, vor allem extremistischer Einstellungen. Im Fokus des Projektes stehen muslimische junge Menschen, die sich radikalisieren bzw. radikalisiert haben. Bestehende Theorien und Daten werden zu Modellen zum kausalen Zusammenhang der Kernkonzepte entwickelt und anhand eines Mixed-Methods-Designs überprüft. Dadurch wird es möglich sein, empirisch fundiert Modelle zu identifizieren, die eine evidenzbasierte Grundlage für die Prävention und Intervention bieten. Die Kernfrage des Kooperationsprojektes „RADIKAL“ ist die Frage nach dem Zusammenhang von Migration, Religion und Radikalisierung. Die theoretische wie die empirische Analyse zielt dabei auf das Verständnis des Zusammenhangs von Migrationserfahrungen, Akkulturationsprozessen, Diskriminierungswahrnehmungen und -erfahrungen und der Bildung von politischen wie religiösen Überzeugungen, hier v.a. extremistischer Einstellungen. Migrationsprozesse werden dabei vor allem auch als Akkulturationsprozesse verstanden, in denen Menschen soziale, politische und religiöse Überzeugungen auf der Grundlage ihrer Erfahrungen während der Akkulturation entwickeln. Es wird angenommen, dass gerade hier extremistische, fundamentalistische und polarisierte (eben radikalisierte) Überzeugungen entstehen können. Auf der Grundlage bestehender Theorien und Daten entwickelt das Projekt zunächst theoretische Modelle zum kausalen Zusammenhang der Kernkonzepte. Diese werden dann anhand einer Kombination von quantitativen wie qualitativen Methoden empirisch untersucht. Dies ermöglicht es, mithilfe der Ergebnisse des Projekts empirisch fundierte Modelle zu identifizieren, die eine evidenzbasierte Grundlage für die Prävention und Intervention bieten. Im Fokus des Projektes stehen muslimische junge Menschen, die sich radikalisieren bzw. radikalisiert haben; sei es in besonders schwerer Weise (Terror) oder weniger schwer (kognitive wie emotionale Nähe zu extremistischen wie fundamentalistischen Gruppen oder Weltanschauungen). Die Aufarbeitung der Analyse soll zudem einen Vergleich mit anderen Phänomenbereichen ermöglichen. Das Projekt besteht aus zwei Phasen: In der ersten Phase wird unabhängig von späteren Einzelfallanalysen kompletter Biografien ein Kodierschema zu den zentralen Konzepten (Migration, Integration/Akkulturation, Diskriminierung, religiöse Bindung und Orientierung) erstellt, um vorhandene und neue Daten zu den Biografien von jungen Menschen zu integrieren. In der zweiten Phase sollen im Besonderen migrations- und integrationsbedingte Risikofaktoren mit Blick auf Radikalisierung und die Bindung an extreme Gruppen herausgearbeitet werden. Auf der Grundlage der Daten der ersten Phase werden dazu hoch radikalisierte Personen mit besonders fundamentalistischer Orientierung identifiziert (qualitative Fallrekonstruktion). Anhand dieser Daten wird eine quantitative Überprüfung der Bedeutung bestimmter Risiko- und Erklärungsfaktoren vorgenommen. Die Ergebnisse von Experteninterviews mit staatlichen und zivilgesellschaftlichen Stakeholdern aus den Bereichen Religion, Sicherheit, Jugendarbeit, Prävention und Deradikalisierung u.a. sollen die Evaluierung der Projekterkenntnisse erweitern und als Grundlage zur Entwicklung von gemeinsamen Strategien der sensiblen Prävention, wie auch neuer Wege der Jugendarbeit dienen."

Das von der DFG geförderte Projekt knüpft an die Forschungsarbeit an, die im Rahmen des deutsch-französischen ANR-DFG-Projekts „Berufliche Strategien und Statuspassagen von jungen Erwachsenen mit Migrationshintergrund im deutsch-französischen Vergleich“ (in Kooperation mit dem Centre Marc Bloch in Berlin) durchgeführt wurde. Das Projekt wird von Dr. Ingrid Tucci und Prof. Dr. Olaf Groh-Samberg geleitet. Ziel ist eine Analyse der Verlaufsmuster junger Menschen mit türkischem Hintergrund, die auf der Kombination von quantitativen und qualitativen Daten im Sinne einer methodologischen und Daten-Triangulation beruht. Hierfür werden qualitative Leitfadeninterviews mit 30 ausgewählten langjährigen Befragten des Sozio-ökonomischen Panels (SOEP) durchgeführt und ausgewertet. Konkret werden Forschungsfragen verfolgt, die sich auf die wechselseitige Validierung quantitativer und qualitativer Befunde, die vertiefende inhaltliche Analyse von Bedingungen und Deutungen sozialer Aufstiege und kultureller Integration und die methodologische Auswertung beziehen. Auch soll eingehender untersucht werden, inwiefern sich biografische Wendepunkte auch in prospektiv erhobenen quantitativen Paneldaten identifizieren lassen. Wendepunkte definieren sich über die Veränderung von Handlungsorientierungen und -strategien, die mit spezifischen Statuspassagen einhergehen oder als Reaktion auf wichtige Ereignisse im Leben auftreten. Eine wichtige Frage betrifft hier auch die Chancen auf (im Lebenslauf) „späte“ soziale Aufstiege nach einer prekären Lebensphase.

Summary in English not available, Summary in Finnish: Hankkeen julkinen kuvaus: Hankkeessa tutkimme terveyttä ja terveysmatkailua Suomessa, Virossa ja Venäjällä käyttäen monia erilaisia tutkimusmenetelmiä (mixed methods). Hankkeen tutkimusprojektit lähestyvät terveysliikkuvuutta maahanmuuton ja terveysturismin näkökulmista. Tutkimme venäläis- ja virolaistaustaisten maahanmuuttajien terveyspalvelujen käyttöä Suomessa ja kotimaissaan sekä venäläisiä ja suomalaisia terveys- ja hyvinvointituristeja Suomessa ja Virossa. Kysymme kuinka käsitykset terveydestä ja hyvinvoinnista muokkautuvat rajat ylittävässä (transnational) kontekstissa ja kuinka nämä käsitykset liikkuvat ja liikuttavat terveyspalveluiden käyttäjiä pohjoisen Itämeren maiden välillä. Olemme kiinnostuneita paikan merkityksestä terveyskäsityksille – kuinka esimerkiksi Suomi määrittyy toisille terveyden turistikohteeksi ja toisille ”epäterveelliseksi” paikaksi, josta halutaan matkustaa tutumpaa, halvempaa tai parempaa palvelua tarjoavaan naapurimaahan. Hankkeemme on rohkea avaus, koska se ylittää rajoja monessa mielessä: ylittämällä metodologisia rajoja (kvalitatiivinen ja kvantitatiivinen tutkimus) sekä ammentamalla eri tieteenalojen teoriaperinteistä (sosiologia, antropologia, terveystutkimus, maantiede ja tilastotiede). Tutkimus pyrkii haastamaan käsityksiämme suomalaisesta terveysjärjestelmästä ja palvelukulttuurista maahanmuuttajien ja muiden rajoja ylittävien käyttäjien näkökulmasta. Hankkeemme on rohkea avaus, koska se ylittää rajoja monessa mielessä: ylittämällä metodologisia rajoja (kvalitatiivinen ja kvantitatiivinen tutkimus) sekä ammentamalla eri tieteenalojen teoriaperinteistä (sosiologia, antropologia, terveystutkimus, maantiede ja tilastotiede).
